Salary: The compensation for this role ranges from CAD $29.38 to CAD $30.96 per hour.

Position Overview:

Join Vancouver Coastal Health (VCH) as a Rehabilitation Support Specialist, where you will be an integral part of the Home Health team. This position involves working collaboratively to support individuals in achieving their rehabilitation goals.

Key Responsibilities:

  • Operate under the guidance of the Manager or Team Leader, following the direction of a licensed occupational therapist or physiotherapist.
  • Deliver assigned therapeutic interventions related to physical and occupational therapy.
  • Assist in the adaptation of care plans and facilitate transitions to community-based programs, promoting client independence.
  • Provide functional guidance to families, document observations regarding client conditions, and manage equipment safety and inventory.

Our Home Health programs offer a variety of healthcare and support services to individuals in the community with diverse health needs, including acute, chronic, palliative, or rehabilitative care.

Client Demographics:

Our clients often face ongoing health challenges and complex personal or social issues, necessitating careful planning and coordination of resources. This includes elderly individuals, younger clients with disabilities, and adults with chronic conditions who require support to maintain or enhance their independence.

Qualifications:

  • Completion of a recognized Diploma-level Rehabilitation Assistant Program with components in Physical Therapy, alongside one year of relevant experience with the target population, or an equivalent combination of education and experience.
  • A valid BC Driver's License is required, as local travel may necessitate the use of a personal vehicle, with mileage reimbursement available.

  • Demonstrated capability in providing self-care instruction and education.

Closing Remarks:

Work hours, including days off and work locations, may vary based on operational needs. Successful candidates may need to undergo a Criminal Records Review Check. As per current health regulations, all employees of Vancouver Coastal Health must be fully vaccinated against COVID-19 or have received the most recent vaccine dose, with proof of vaccination required.
