Job Summary:
Fraser Health Authority is seeking a Registered Nurse to coordinate the Colon Screening Program. The successful candidate will work collaboratively with an integrated team to provide patient-centered care and support.
Key Responsibilities:
- Coordinate care for program participants/patients referred by physicians to the Colon Screening Program across various sites.
- Perform pre and post-colonoscopy assessments, provide patient and family education, and ensure continuity of care.
- Collaborate with the interdisciplinary team to develop and implement care plans, provide direct care, and intervene as required.
- Assess patient and family needs for education and support, and provide information, education, and support regarding screening, follow-up procedures, risks, and benefits.
- Develop, deliver, and evaluate patient education materials, and initiate referrals to the interdisciplinary team as appropriate.
- Support and coordinate the work of the clinical team for designated clinics/hospitals through coordination of assessment, education, and booking processes.
- Conduct post-colonoscopy assessments, review pathology with colonoscopists, and recommend further testing as required.
- Follow up with patients, complete follow-up forms, and submit information to the BC Cancer Agency for purposes of findings and surveillance recommendations.
- Collect, compile, and submit data to the BC Cancer Agency, and ensure quality assurance guidelines/parameters are adhered to in the collecting and reporting of information/data.
- Participate in the development of program policies, procedures, standards, and processes for colon screening and follow-up, and evaluate and monitor service outcomes.
- Participate in the ongoing evaluation, assessment, and improvement of the Colon Screening Program through close liaison with the Manager and integrated team.
Qualifications:
- Graduation from an approved School of Nursing.
- Two (2) years recent, related experience working with patients undergoing diagnostic or surgical procedures for diagnosis or treatment for colon cancer screening or an equivalent combination of education, training, and experience.
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Valid BC Driver's License with access to a vehicle for business-related purposes.
