**Job Summary:**
We are seeking a highly skilled and experienced Nurse Practitioner to join our Renal Program at William Osler Health System. The successful candidate will be responsible for providing high-quality patient care, collaborating with interdisciplinary teams, and contributing to the development of clinical policies and procedures.
Key Responsibilities:
- Provide comprehensive care to patients with chronic kidney disease (CKD), including assessment, diagnosis, and management of their condition.
- Collaborate with nephrologists, primary care providers, and other healthcare professionals to ensure seamless transitions of care and optimal patient outcomes.
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Conduct regular rounds in in-centre hemodialysis pods to address immediate health needs and provide education to patients and families.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Contribute to the development of clinical policies and procedures related to renal patient care.
- Collaborate with other Nurse Practitioners and healthcare professionals to advance knowledge and best practices in renal care.
Requirements:
- Master's degree in Nursing with a focus on nephrology or a related field.
- Current RN (EC) registration with the Ontario College of Nurses and certification as a nephrology Nurse (C.Neph).
- Minimum 2 years' experience in the care of patients with CKD and associated comorbid conditions.
- Strong knowledge of chronic disease prevention and management and experience with hospital nursing functions and current needs in nursing and the healthcare system.
- Excellent decision-making, critical thinking, and problem-solving skills with the ability to exercise independent judgment and assess situations efficiently and effectively.
- Effective organizational, conceptual, and leadership skills with the ability to organize workload and establish priorities effectively.
- Excellent negotiation and conflict resolution skills with the ability to build and maintain effective relationships with a diverse range of people.
- Motivated self-directed learner with a commitment to ongoing education and professional development.
