The Licensed Practical Nurse (LPN) is a key member of our care team, responsible for coordinating the delivery of care to our residents. As a role model and team leader, you will be responsible for ensuring that our residents receive high-quality, resident-centered care.
Responsibilities- Assess residents' needs and implement individualized care plans
- Ensure residents' rights are observed and adhere to a holistic philosophy of wellness
- Liaise with physicians, community agency representatives, families, visitors, volunteers, and other disciplines to ensure quality care is delivered
- Administer prescribed medications and treatments within the limitations of the BC College of Nurses and Midwives (BCCNM)
- Monitor and report changes in physical and psychological conditions of residents
- Document changes on the resident health record and notify family and physician of changes in condition
- Participate in Interdisciplinary Care Conferences and care plan reviews
- Support administrative requirements at the community, including admissions and confidentiality
- Actively participate in orientation of new staff, community programs, meetings, committees, and educational in-services
- Provide coaching and direction to care staff as needed
- Graduate of an approved program for Practical Nursing and holds a current license with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN
- Valid Occupational First Aid Level 1 certification
- Successful annual Resident Assessment Instrument (RAI) competency testing
- Previous experience in a long term care facility is an asset
- Ability to read, write, and speak English proficiently and understand verbal and written instructions
- Computer skills including the ability to navigate basic computer programs such as Word, Excel, Outlook
- Ability to push, pull, stoop, crouch, lift and carry up to 25 lbs
