About the Role

VON Canada is seeking a skilled Licensed Practical Nurse to join our team in providing high-quality home care services to our clients. As a LPN, you will be responsible for delivering patient-centered care in a home setting, working collaboratively with our interdisciplinary team to ensure the best possible outcomes for our clients.

Key Responsibilities
  • Client Care
    • Provide comprehensive nursing care to clients in their homes, including assessments, medication management, and wound care.
    • Develop and implement individualized care plans in collaboration with clients, families, and healthcare professionals.
  • Communication and Collaboration
    • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
    • Collaborate with the interdisciplinary team to share knowledge, expertise, and best practices.
  • Documentation and Reporting
    • Accurately document client information, including assessments, care plans, and progress notes.
    • Prepare and submit reports as required, including incident reports and quality improvement initiatives.
  • Professional Development
    • Participate in ongoing education and training to enhance clinical skills and knowledge.
    • Stay current with best practices, research, and industry developments.
Requirements
  • Education and Certification
    • Current licensure as a Licensed Practical Nurse (LPN) in the province of employment.
    • Completion of a recognized LPN program.
  • Experience and Skills
    • Minimum 1 year of experience as an LPN in a home care or visiting care agency setting.
    • Excellent communication and interpersonal skills.
    • Ability to work independently and as part of a team.
    • Strong problem-solving and critical thinking skills.
    • Proficiency in electronic medical records and other software applications.
