Job Overview

The Patient Care Coordinator for Ambulatory Services plays a crucial role within the clinical management team, providing support across various facilities. This position is accountable for the comprehensive daily coordination of care for patients and their families, ensuring efficient patient flow through the ambulatory program. The coordinator will liaise with clinical staff and resources, serving as a vital clinical resource for physicians, nurses, and the interprofessional team. Additionally, the role encompasses supervisory responsibilities, including attendance management, performance evaluation, and conflict resolution at the point of care and beyond. The coordinator will assist physicians in navigating services for primary care and Long Term Care Homes, ensuring centralized access to hospital and community resources, while following up on referrals, results, and consultations. This position is essential for facilitating timely access to care, directing patients to appropriate resources, and minimizing emergency department visits by proactively connecting patients to the necessary level of care.

Required Qualifications

Registered Nurse with a Bachelor of Science in Nursing (BScN) or currently pursuing the degree. Valid general certificate of registration and a member in good standing with the College of Nurses of Ontario. Current Basic Cardiac Life Support (BCLS) certification is mandatory.

Preferred Qualifications

At least two years of relevant experience in acute inpatient care. Familiarity with Quality-Based Procedures (QBP) pathways and management of this patient demographic. Experience in case management and discharge planning. Demonstrated understanding and application of quality care principles and outcome measurement. Ability to identify relevant information for effective decision-making regarding patient care and program planning. Knowledge of CNO standards of practice and an understanding of the RHPA legislation. Capacity to work independently and lead inter-professional clinical teams effectively. Strong conflict resolution skills. Well-developed skills in task delegation. Commitment to ongoing professional development and learning. Excellent interpersonal, communication, coaching, mentoring, and team-building skills that promote positive interactions among team members. Understanding of continuous improvement principles and tools. Awareness of contributions to patient and staff safety. Highly developed analytical, problem-solving, and critical thinking abilities. Strong patient and customer-focused approach in all interactions. Ability to facilitate and manage change effectively. Proven leadership skills in providing support and critical analysis in complex clinical situations. Competence in analyzing research and implementing evidence-based practices into policy and clinical practice. Experience in guiding staff with performance and attendance challenges. Effective communication skills, both written and verbal, ensuring clarity and understanding. Proficient in utilizing effective listening skills to enhance understanding of issues. Proven ability to work within a computerized environment. Strong attendance record. Understanding and compliance with safety policies and procedures that create a safe and inclusive environment.
