Job Title: Case Worker

Job Summary:

We are seeking a skilled and compassionate Case Worker to join our team at the Centre for Addiction and Mental Health. As a Case Worker, you will play a vital role in supporting individuals with complex mental health needs, including those with dual diagnosis disorders.

Key Responsibilities:

  • Provide individualized support to clients with dual diagnosis disorders, developmental disorders, and other complex mental illnesses.
  • Work collaboratively with interdisciplinary teams to develop and implement personalized care plans.
  • Support clients with daily living activities, social skills building, and facilitate interventions to promote recovery and independence.
  • Participate in clinical team reviews, quality improvement initiatives, and program development.
  • Engage in clinical documentation and other duties as assigned.

Requirements:

  • Developmental Service Worker diploma from a recognized college.
  • Direct experience working with clients with dual diagnosis disorders.
  • Knowledge of behavioral treatment protocols, crisis prevention, and intervention skills.
  • Ability to work in a biopsychosocial model of care.
  • Strong interpersonal and communication skills.
