Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Nurse. As a Home Health Nurse, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Develop and implement individualized patient care plans that meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and emotional needs, and provide education and support to patients and their families.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide wound care, medication management, and other nursing interventions as needed.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Current RN licensure in the state of employment.
  • Minimum of 6 months of recent experience as a home care nurse or equivalent experience in a related field.
  • Graduate of an approved School of Nursing.
  • Current CPR certification.
  • Ability to lift, push, pull, and carry a minimum of 50 lbs.
  • Ability to stoop and bend up to 2.5 hours per 8-hour shift.
