About the Role:
We are seeking a skilled Clinical Social Worker to join our interdisciplinary team. As a key member of our team, you will provide high-quality social work services to improve the emotional and social consequences of health conditions for our patients and families.
Key Responsibilities:- Conduct comprehensive social and psychosocial assessments of patients and families, and interpret the information using a variety of modalities.
- Provide therapeutic counseling services to clients and families experiencing a range of circumstances related to their illness.
- Facilitate a process whereby the patient and family identify their needs for recovery, adaptation, and mastery of the situation.
- Develop and implement comprehensive plans of care with/for the client, and collaborate and consult with other community professionals.
- Act as a resource for clients and their families by providing information regarding community resources and access to these resources.
- Help clients explore their reactions to loss and provide counseling from a strengths perspective to sustain client motivation and hope.
- Coordinate and advocate for the client with other service providers to promote maximum client social functioning.
- Participate in quality improvement activities by providing input into the development, implementation, and evaluation of program goals, objectives, policies, and procedures.
- Educate other health service providers about the psychosocial aspects of the health problem through participation at discharge planning rounds, patient and family conferences, clinical documentation, and consultations.
- Document clinical interventions and participate in outcome assessments and workload measurement activities.
- Bachelor's degree in Social Work from an approved school of Social Work.
- Two (2) years' recent related clinical social work experience working with individuals and families with mental health and substance use issues.
- Current full registration with the BC College of Social Workers.
- Valid BC Driver's License and access to personal vehicle for business-related purposes.
