About the Role:
Closing the Gap Healthcare Group is seeking a highly organized and detail-oriented Care Team Coordinator to join our team. As a Care Team Coordinator, you will be responsible for scheduling multiple workers with a large number of clients in a fast-paced environment.
Key Responsibilities:
- Advanced organizational skills, scheduling, and multi-tasking are required for this position.
- You will be responsible for accurately and efficiently assessing customer requests and addressing them immediately or escalating as appropriate.
- Scheduling and coordination of service providers to meet client needs.
- Strong problem-solving skills and effective communication with team members.
- Providing CTG program and service information to customers to encourage sales of services.
- Performing data entry requirements for all clients using the CTG computer system.
