About This Role

As a Case Manager at The Salvation Army, you will play a vital role in supporting individuals experiencing homelessness and working towards their housing stability.

Your Responsibilities:

  • Provide personalized guidance and support to residents of our shelter, connecting them with essential resources and services.
  • Conduct thorough intake interviews to understand individual needs and develop tailored case plans focused on achieving housing goals.
  • Collaborate with residents to set achievable objectives, monitor progress, and celebrate successes along their journey towards stable housing.
  • Leverage your knowledge of community resources to connect residents with appropriate support systems for housing, employment, education, recovery, and other essential needs.
  • Maintain accurate and confidential case records, ensuring compliance with established protocols and best practices.
  • Participate actively in case management meetings and contribute to a collaborative team environment.
  • Build strong relationships with community partners to expand access to vital services for our residents.

Your Qualifications:

  • A minimum of two years of experience in the mental health or addiction field, demonstrating a commitment to serving vulnerable populations.
  • Strong understanding of community resources and a proven ability to connect individuals with appropriate support systems.
  • Excellent communication skills, both written and verbal, enabling you to effectively interact with residents, colleagues, and community partners.
  • A compassionate and empathetic approach, coupled with the ability to work independently and as part of a team.
