About the Role:
Closing the Gap Healthcare Group is seeking a skilled Occupational Therapist to join our team of healthcare professionals. As an Occupational Therapist with our company, you will have the opportunity to work with a diverse range of clients in their own homes, providing personalized therapy services to help them achieve their goals.
Key Responsibilities:
- Assess and develop individualized therapy care plans for clients in their homes
- Provide a range of therapy services, including health promotion programs, equipment prescription, and client safety promotion
- Collaborate with healthcare teams and family members to develop holistic treatment goals and service plans
- Work independently and flexibly, using a hybrid model of care that includes face-to-face and virtual interactions
Requirements:
- Graduate of a recognized Post-Secondary program in Occupational Therapy
- Current active registration and good standing as an Occupational Therapist with the College of Occupational Therapists of Ontario
- Valid Driver's License and reliable vehicle
