We are seeking a skilled and compassionate Registered Nurse to join our team at Good Shepherd Centres. As a Palliative Care Nurse, you will play a vital role in providing high-quality care to individuals and their families in our hospice setting.
Key Responsibilities- Provide direct patient care, including assessment, planning, implementation, and evaluation of care
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Conduct clinical assessments and develop treatment plans to manage pain and symptoms
- Provide education and support to patients and families on end-of-life care and advance care planning
- Participate in quality improvement initiatives to enhance patient care and outcomes
- Graduate of an accredited school of nursing and registered in good standing with the College of Nurses of Ontario
- BCLS certificate required or willingness to obtain upon hire
- Experience working with individuals and their families in end-of-life or life-limiting illness settings
- Strong clinical assessment skills and ability to work with evidence-based practices related to pain and symptom management
- Previous palliative care experience in hospital-based palliative care, hospice, or other healthcare settings
- Proven ability to lead and work collaboratively as a member of an interdisciplinary team
