We are seeking a highly skilled Social Worker to join our Interprofessional Primary Care team at the Hamilton Family Health Team. As a key member of our team, you will play a vital role in providing clinical support to unaffiliated physicians and their patients, while also contributing to the expansion of our team-based care services.
Key Responsibilities- Perform comprehensive biopsychosocial assessments and develop and implement individualized treatment plans.
- Provide short-term treatment interventions, including counseling and therapy, to patients and their families.
- Collaborate with patients, primary care providers, and interprofessional teams to monitor and adapt treatment plans as needed.
- Participate in the development and execution of health promotion activities to increase outreach and patient enrollment.
- Assist patients and providers in navigating the healthcare system to identify and connect them with appropriate services and supports.
- Participate in team building, program development, and quality improvement activities to enhance the delivery of care.
- Advocate for patients and their families in matters affecting their health and well-being.
- Liaise and build relationships with other primary care providers, community agencies, and healthcare organizations to ensure seamless care coordination.
- Teach family medicine residents and medical students about the role of the social worker and psychosocial issues facing patients and families.
- Provide evidence-informed mental health interventions to patients and families.
- Develop and facilitate evidence-informed treatment groups and patient education groups to meet patient and system needs.
- Master of Social Work degree from an accredited institution.
- Membership in good standing with the Ontario College of Social Workers and Social Service Workers.
- Professional liability insurance.
- Experience working in healthcare settings, delivering social work and mental health services.
- Experience working with multiple partners in healthcare and community settings.
- Experience in intervening on social determinants of health and mental health.
- Experience working in interprofessional teams and healthcare settings.
- Health promotion experience preferred.
