Your Opportunity:

This engaging position as a Registered Nurse, Designated Float, addresses both scheduled and unscheduled staff shortages. Alberta Health Services employs a strengths-based Client-Centered Case Management practice framework. By utilizing a person-focused approach, you will create a care plan that empowers the client and their family to recognize needs, emphasize strengths, establish attainable personal health and self-care objectives, and deliver direct nursing care. Collaborating with the client, their family, and the interdisciplinary team, you will ensure that services are continuously adapted based on the client's responses to interventions, incorporating stewardship into service authorization. You will report to the Home Care Manager and be accountable for coordinating and delivering high-quality care to clients within the community. Your responsibilities will include conducting nursing assessments, coordinating cases, and providing direct care in partnership with both short-term and long-term clients and their families in their own homes or alternative care environments, as determined by assessments. You will work across various teams.

Description:

As a Registered Nurse (RN), you will deliver a comprehensive range of nursing services to patients, families, communities, and populations, while taking essential measures to ensure their safety and well-being. In this role, you will apply nursing processes through critical thinking, problem-solving, and decision-making, as well as educate, counsel, and advocate for patients and their families. You will assume a leadership role in fostering an integrated and holistic approach to patient care, health promotion, and maintenance. Your commitment will be to provide safe, quality, patient- and family-centered care while embodying the shared vision and values of Alberta Health Services.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters and the NextGenU Health Assessment Resources Certification.

Additional Required Qualifications: Preferred Qualifications:

The ideal candidate will possess: a Bachelor's degree in Nursing; experience working with acute and/or long-term clients focusing on chronic disease management; experience in case management, client education, and professional nursing interventions, including but not limited to Home Parenteral Therapy (HPT), wound care, and ostomy care; experience supporting vulnerable clients requiring palliative, mental health, and/or addiction support.
