Job Summary

Island Health is seeking a highly skilled Occupational Therapist to join our team as a Community Care Coordinator. This is an exciting opportunity to work with clients with complex disabilities and make a meaningful difference in their lives.

Key Responsibilities
  • Client Assessment and Care Planning: Conduct comprehensive assessments of clients' physical, mental, and functional abilities to develop personalized care plans.
  • Community Health Experience: Utilize your expertise in community health to provide home care services and support clients in their own environments.
  • Collaboration and Communication: Work effectively with clients, families, and healthcare teams to ensure seamless care coordination and delivery.
  • Case Management and Coordination: Develop and implement discharge plans, coordinate services, and ensure timely and effective transitions.
  • Professional Development: Stay up-to-date with industry developments, best practices, and evidence-based research to enhance your skills and knowledge.
Requirements
  • Education and Certification: Graduation from a recognized university program in Occupational Therapy and current practicing full registration with the College of Occupational Therapists of British Columbia (COTBC).
  • Experience: Three (3) years' recent related experience in working with clients with complex disabilities, including two (2) years' community health experience with a home care component.
  • Skills and Abilities: Demonstrated ability to integrate and apply COTBC Standards for Occupational Therapy Practice, working knowledge of community health, and excellent communication and interpersonal skills.
