Clinical Nurse Coordinator

Position Overview: Nursing Case Manager
The Nursing Case Manager is responsible for orchestrating, organizing, and directing hospice services with a focus on community health education and engagement. This role emphasizes leveraging community resources to address the unique needs of patients and their families within their homes and local environments.
KEY RESPONSIBILITIES
Patient Care Management
- Conducts thorough initial evaluations of patient and family requirements.
- Delivers expert nursing care utilizing the nursing process.
- Creates care strategies based on nursing assessments and integrates palliative care interventions.
- Oversees patient care for a designated caseload.
Effective Communication
- Maintains precise clinical documentation and collaborates with healthcare team members.
- Educates patients and families on self-care practices.
- Participates in interdisciplinary team meetings.
Additional Responsibilities
- Engages in on-call responsibilities as outlined.
- Supervises support staff and delegates tasks as necessary.
- Involves in ongoing education and professional growth opportunities.

Qualifications
- Graduate from an accredited nursing program with experience in acute care settings.
- Current nursing license in the relevant state.
- Excellent communication and problem-solving abilities.
- Physical capability to assist patients and manage medical equipment.
- Must possess a valid driver's license and have access to reliable transportation.
