Position Overview: Registered Nurse Case Manager
The Registered Nurse Case Manager is responsible for the planning, organization, and direction of hospice services, focusing on community health education and experience. This nursing professional utilizes community resources to address the needs of individuals and families within their homes and communities.
KEY RESPONSIBILITIES
Patient Care Management
- Conducts thorough initial evaluations of patient and family requirements.
- Delivers professional nursing care through the application of the nursing process.
- Creates care plans based on nursing assessments and integrates palliative care strategies.
- Oversees patient care for an assigned group of patients.
Effective Communication
- Keeps precise clinical documentation and collaborates with healthcare team members.
- Educates patients and families on self-care practices.
- Participates in interdisciplinary team meetings.
Additional Responsibilities
- Engages in on-call responsibilities as required.
- Supervises support staff and assigns tasks as necessary.
- Involves in ongoing education and professional growth.

Qualifications Required
- Graduate from an accredited nursing program with experience in acute care.
- Current nursing license in the relevant state.
- Excellent communication and problem-solving abilities.
- Physical capability to assist patients and manage medical equipment.
- Must possess a valid driver's license and have access to reliable transportation.
