Job Summary:

We are seeking a skilled and compassionate Nurse Care Specialist to join our team at ProVita Care Management. As a key member of our healthcare team, you will play a vital role in providing high-quality care to our residents in a dynamic and supportive environment.

Key Responsibilities:

  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Develop and implement individualized care plans to meet the unique needs of each resident.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Conduct thorough assessments and evaluations to identify resident needs and preferences.
  • Document resident progress and care plans in a timely and accurate manner.

Requirements:

  • Graduation from a recognized Nursing program or equivalent combination of education, training, and experience recognized by the BCCNM.
  • Maintain Occupational First Aide Level 1 status.
  • Satisfactory proof of COVID-19 vaccinations.
  • Ability to effectively communicate in English, both verbal and written.
