Chronic Disease Nurse, Primary Care Specialist

Fraser Health Authority is seeking a skilled Chronic Disease Nurse to join our team in providing high-quality care to patients with chronic conditions. As a member of our multidisciplinary team, you will play a crucial role in developing and implementing individualized care plans, teaching patients and families about chronic disease management, and collaborating with other healthcare professionals to ensure optimal health outcomes.
Key Responsibilities- Develop and Implement Care Plans: Develop, implement, and evaluate individualized care plans for patients with chronic diseases, in collaboration with other healthcare professionals.
- Teach and Educate Patients: Enhance patient self-management of chronic diseases through various teaching and education methods, including individual counseling and group facilitation.
- Conduct Assessments and Share Information: Conduct outpatient assessments, share information with other healthcare team members, and collaborate to ensure the provision of quality chronic disease management services.
- Provide Orientation and Mentoring: Provide orientation and mentoring to new staff, and participate as a preceptor to students.
- Participate in Quality Improvement: Participate in quality improvement and risk management activities, including research and special projects related to chronic disease management.
Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM) and current CPR certification. Two (2) years' recent related clinical experience treating and assessing patients with chronic diseases within a chronic disease management framework, including patient self-management.
Skills and Abilities
- Demonstrated knowledge of current standards for chronic disease management.
- Demonstrated understanding of the principles of adult/child/youth learning, including strategies around empowerment and motivation.
- Ability to communicate effectively, both verbally and in writing.
- Ability to work independently and as a member of a multidisciplinary team.
- Demonstrated ability to teach and implement a chronic disease education program for patients/clients/families, in both an individual and group setting.
- Ability to create and maintain rapport with patients/clients/families and with other healthcare providers.
- Ability to plan and prioritize work.
- Ability to operate related equipment, including applicable software applications.
- Physical ability to perform the duties of the position.
