Key Responsibilities- Provide Patient-Focused Care: Deliver safe, holistic, and patient-centered nursing care, working collaboratively with other healthcare professionals to achieve successful outcomes.
- Assess and Respond to Patient Needs: Continuously assess, anticipate, identify, and respond to the changing needs of patients, ensuring their care is based on best practices.
- Travel and Communication: Travel within your region to patients' homes, maintaining effective communication with patients, families, and healthcare teams.
- Experience and Education: Minimum two years of nursing experience, with a strong preference for community nursing experience. Registered and in good standing with the College of Nurses of Ontario (CNO).
- Skills and Abilities: Ability to work independently in the community, proficiency in English (both verbal and written), and a valid Ontario driver's license with a reliable vehicle.
- Specialized Skills: Experience with infusion therapy, mental health, palliative care, and wound care, as well as excellent patient assessment and teaching skills.
