Job Summary

We are seeking a highly motivated and compassionate Life Skills Support Worker to join our Primary Care team at Northern Health Authority. As a key member of our interprofessional team, you will play a vital role in providing exceptional care and support to individuals with complex health issues.

Key Responsibilities
  • Apply best practice to provide individuals with services outlined in their plan of care, with the goal of maximizing independent living.
  • Work collaboratively with the interprofessional team to provide comprehensive care and support.
  • Develop and maintain rapport with individuals and their families to ensure their needs are met.
  • Teach skills to support the highest level of function possible and promote independence.
  • Contribute to the development and implementation of individualized care plans.
Requirements
  • Diploma from a recognized Social Services Support Worker educational program, or Residential Care Aide program, two (2) years' recent, related experience working with people with complex health issues, or an equivalent combination of education, training and experience.
  • Valid driver's license.
  • Valid First Aid Certificate, CPR and Food Safe.
  • Ability to work independently and in collaboration with others.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to organize and prioritize.
  • Ability to observe and recognize changes in a person's condition or health status.
