We are seeking a highly skilled and compassionate Indigenous Mental Health Liaison Nurse to join our team at Fraser Health. As a key member of our multi-disciplinary team, you will play a vital role in providing culturally sensitive nursing and intermediary services to First Nations, Métis, and Inuit clients.
Key Responsibilities- Client Advocacy and Support: Act as a client advocate, clarifying expectations and providing support to clients and their families.
- Assessment and Referral: Conduct client assessments, identify needs, and refer clients to appropriate services, ensuring seamless transitions and optimal care.
- Health Promotion and Education: Provide education and promote health promotion initiatives to individuals, families, and Indigenous communities, empowering them to maintain holistic wellness.
- Community Liaison and Navigation: Act as a point of contact, assisting clients with health system navigation and liaising with programs and services across Fraser Health and community partners.
- Cultural Competence and Safety: Provide culturally sensitive services, promoting cultural competence and safety within our organization.
- Communication and Collaboration: Communicate effectively with clients, families, healthcare professionals, and community agencies, fostering strong relationships and collaborative partnerships.
- Record Keeping and Reporting: Maintain accurate records and prepare reports as required, ensuring compliance with established standards and procedures.
- Education and Experience: Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM), with a minimum of two years' recent, relevant nursing experience.
- Knowledge and Skills: Demonstrated knowledge of nursing process, epidemiology, family health, health promotion, and community development, as well as the ability to provide consultation, make effective referrals, and communicate effectively.
- Cultural Competence: Comprehensive knowledge of Indigenous culture and community resources, with the ability to deliver culturally sensitive services to Indigenous populations.
