Job Title: Social Worker, MSW - Inpatient Adult Mental Health

Job Summary:

Humber River Health is seeking a highly skilled and compassionate Social Worker, MSW to join our Inpatient Adult Mental Health team. As a Social Worker, you will play a vital role in providing psycho-social assessment and therapies to individuals, families, and groups, promoting an optimum level of functioning and supporting patients during all phases of recovery from usage to abstinence.

Key Responsibilities:

  • Completes screening and detailed addiction assessment for all referred patients
  • Provides psycho-social assessment and therapies using a variety of psychotherapeutic modalities
  • Supports patients during all phases of recovery from usage to abstinence
  • Develops and coordinates discharge plans in conjunction with the treatment team
  • Completes patient documentation, workload measurements, and data requirements

Requirements:

  • MSW obtained from an accredited School of Social Work
  • Current membership with OASW and OCSWSSW
  • Two years' experience in mental health and/or addiction facility or setting preferred
