Position Overview

The Resource Coordinator for Complex Special Needs plays a pivotal role in ensuring the effective delivery of services to eligible children. Reporting to the Supervisor of Complex Special Needs, this position emphasizes a person-centered methodology, overseeing the creation and execution of evidence-based service plans tailored to individual requirements.

Key Responsibilities
  • Engage with individuals and their families to assess current and future service options, linking them to necessary resources while participating in the goal-setting process through personalized care plans.
  • Develop comprehensive Person-Centered Plans in collaboration with children, families, and support networks to identify specific support needs.
  • Conduct annual reviews of service plans and documentation as mandated.
  • Ensure compliance with all required updates and reviews by specified deadlines.
  • Provide essential resource information to individuals and families.
  • Establish and nurture positive working relationships with all stakeholders.
  • Conduct regular visits to monitor service provision, both scheduled and unscheduled.
  • Address any incidents or serious occurrences related to care, ensuring appropriate follow-up actions are taken.
  • Participate in clinical consultations, school meetings, and other relevant discussions regarding care provision.
  • Manage financial forecasting and contribute to budget development.
  • Report any risks associated with individual care to the Supervisor.
  • Collaborate with clients and stakeholders to resolve issues as necessary.
  • Facilitate financial discussions in accordance with departmental policies and procedures.
  • Maintain the accuracy and integrity of client files and agency databases in line with established standards.
  • Attend regular supervision sessions to review case loads and receive guidance.
  • Adhere to all policies and procedures of the service coordination framework.
  • Promote awareness of services by participating in presentations to community groups and other agencies as needed.
  • Perform additional duties as assigned.
Qualifications and Skills
  • Minimum of three years of experience in the developmental services sector.
  • A Bachelor's degree in Social Work or a related field is required.
  • Strong interpersonal and communication skills.
  • Proficient analytical, problem-solving, and critical thinking abilities.
  • Familiarity with available services for individuals with developmental disabilities.
  • Experience in case management.
  • Knowledge of relevant legislation and regulations is essential.
  • Excellent organizational and time management skills to meet deadlines.
  • Able to thrive in a fast-paced environment while managing multiple priorities.
  • Proficient in Microsoft Office Suite and other relevant software.
  • Possession of a valid driver's license and access to a vehicle for work-related travel.
Work Environment

The role involves significant interaction within community settings, including family homes and service locations, necessitating local travel. Work schedules and priorities are typically established in advance.
