We are seeking a highly skilled and compassionate Transitional Support Specialist to join our team at Women's Habitat of Etobicoke. As a key member of our outreach team, you will work closely with women who have experienced abuse, providing them with the support and resources they need to achieve independence and stability in their lives.

Key Responsibilities
  • Assess the needs of clients and develop personalized transitional plans to access community supports
  • Provide supportive counseling, outreach, and advocacy services to women in need
  • Liaise and coordinate with community resources to ensure seamless support for clients
  • Assist clients in accessing services such as counseling, education, job training, and income support resources
  • Work collaboratively with the outreach team to ensure comprehensive service delivery
Requirements
  • Diploma or degree in Social Work or a related field
  • Knowledge of crisis intervention techniques and community resources
  • Sensitivity and awareness of cultural, racial, economic, and socially diverse communities
  • Ability to administer Naloxone harm reduction supports
  • Second language an asset
