Program Overview:

The LEAD (Let Everyone Advance with Dignity) initiative is a collaborative effort designed to enhance community capacity for improving public health and safety while reducing dependence on law enforcement and the judicial system in addressing public order challenges associated with substance use and sex work.

Role Summary:

The Community Outreach and Screening Specialist serves as the primary contact for all referrals to the LEAD case management system. This role is crucial for ensuring effective communication and collaboration among all stakeholders involved in the referral process. Key responsibilities include:

  • Acting as the main point of contact for all LEAD referrals.
  • Conducting initial screenings for new referrals and addressing immediate client needs.
  • Engaging in street outreach to connect with individuals referred to the LEAD program.
  • Coordinating the assignment of new referrals to appropriate case managers.
  • Building and maintaining strong, collaborative relationships with LEAD partners and other service providers.
  • Documenting all activities in accordance with agency and contractual obligations.

Qualifications:

A high school diploma or equivalent is required; an undergraduate degree is preferred. Relevant experience may be considered in lieu of a degree.

Essential Skills:

  • Proven experience in providing outreach and engagement services in community settings.
  • Knowledge of substance use disorders and harm reduction methodologies.
  • Strong communication and interpersonal skills.
  • Excellent organizational abilities with a capacity to manage multiple priorities.

Working Conditions:

Employees may encounter exposure to illicit substances and related fumes. The role may require occasional lifting or moving of items weighing up to 30 pounds.
