About the RoleAs a Registered Practical Nurse, you will be responsible for providing high-quality patient care in a community setting. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each client.
Responsibilities- Conduct comprehensive client assessments and develop care plans that address the physical, emotional, and social needs of each client.
- Implement and evaluate care plans in collaboration with the client, family, and healthcare team.
- Provide education and support to clients and their families to promote independence and self-care.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal client outcomes.
- Current registration with the College of Nurses of Ontario.
- Canadian Nurses Association (CNA) specialty certification an asset.
- Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care.
- A current CPR certificate is required.
- A valid driver's license and vehicle (travel required).
- Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
