Clinical Supervisor

We are seeking a highly skilled and experienced Clinical Supervisor to join our team at ParaMed Home Health Care. As a Clinical Supervisor, you will be responsible for the supervision of Personal Support Workers (PSWs) and Health Care Aides (HCAs) who provide care and support to clients in their homes.
Key Responsibilities- Supervise and support PSWs and HCAs to ensure they are providing high-quality care and services to clients.
- Respond to and resolve concerns from clients, families, and other team members.
- Provide ongoing leadership and guidance to PSWs and HCAs through regular meetings and one-on-one sessions.
- Investigate and document patient safety incidents to ensure timely action plans are implemented.
- Collaborate with the nursing team to support clinical documentation and submission requirements.
- Engage clients in problem-solving and health teaching to identify their needs, strengths, and goals.
- Engage staff in problem-solving and provide education, coaching, and mentoring to resolve issues.
- Maintain knowledge and skills to provide safe, efficient, and effective care by staying up-to-date with changes in practice settings.
- Current RN or RPN registration with the College of Nurses of Ontario and entitlement to practice without restrictions.
- Minimum of 5 years' experience as a practicing Registered Nurse or Registered Practical Nurse.
- Comprehensive patient health assessment skills.
- Understanding of community nursing practice principles.
- Proven experience leading and managing a team.
- Knowledge of clinical documentation standards and practice guidelines.
- Excellent organizational, interpersonal, and teaching skills.
- Commitment to professional development in clinical practice relevant to home health care.
- Effective critical thinking and problem-solving skills.
- Valid driver's license and access to reliable transportation.
- Able to provide a current, clear Vulnerable Sector Screening (VSS).
- C.N.A. Certification as a Community Health Nurse (CCHN(C)) is considered an asset.
