Key Responsibilities:

  • Collaborate with a multidisciplinary team, including government agencies, healthcare professionals, and community resources
  • Demonstrate commitment to continuous learning and professional growth through participation in conferences, committees, and networks

Qualifications:

  • Active registration as a Registered Nurse with the provincial regulatory body
  • Bachelor of Science in Nursing (BScN) preferred
  • At least three (3) years in a formal nursing leadership position required
  • Minimum of two years' experience as a practicing Registered Nurse in a community setting
  • Certification in Community Health Nursing with CNA certification preferred
  • Membership with the Registered Nurses Association of Ontario (RNAO) preferred
  • Familiarity with relevant healthcare legislation, including the Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, and Privacy/Personal Health Information Act
  • Knowledge of care standards for palliative, wound, IV therapy, and other specialties to provide guidance on best practices
  • Excellent verbal and written communication skills to effectively engage with clients, families, contractors, and staff to resolve conflicts
  • Strong critical thinking and problem-solving abilities
  • Flexibility in work hours to meet operational demands
  • Awareness of community resources available for clients and families
  • Current certification in First Aid and CPR, completed within Canada within the last 12 months
  • Proficient computer skills, including experience with Word and Excel
  • Ability to provide a current, clear criminal background check with Vulnerable Position Screening (VPS)
