We are seeking a highly skilled and experienced Clinical Supervisor to join our team at ParaMed Home Health Care. As a Clinical Supervisor, you will be responsible for the supervision of Personal Support Workers (PSWs) and Health Care Aides (HCAs) who provide care and support to clients in their homes.
Key Responsibilities- Supervise and support PSWs and HCAs to ensure they are providing high-quality care that meets the needs of our clients.
- Respond to and resolve concerns from community staff, patients, and families in a timely and professional manner.
- Provide ongoing leadership and guidance to PSWs and HCAs through regular meetings, training, and coaching.
- Initiate and lead patient safety incident investigations and ensure that action plans are implemented to prevent future incidents.
- Work collaboratively with the nursing team to support clinical documentation and submission requirements.
- Engage patients and families in problem-solving and health teaching to identify their needs, strengths, and goals.
- Engage staff in problem-solving and provide education, coaching, and mentoring to resolve issues.
- Maintain up-to-date knowledge and skills to provide safe, efficient, and effective care in all areas of practice.
- Current RN or RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions.
- Minimum of 5 years' experience as a practicing Registered Nurse or Registered Practical Nurse.
- Comprehensive patient health assessment skills.
- Understanding of the principles of community nursing practice.
- Proven experience leading and managing a team.
- Knowledge of clinical documentation standards and practice guidelines.
- Excellent organizational, interpersonal, and teaching skills.
- Commitment to professional development in clinical practice relevant to home health care.
- Effective critical thinking and problem-solving skills.
- Valid driver's license and access to reliable transportation.
- Able to provide a current, clear Vulnerable Sector Screening (VSS).
- C.N.A. Certification as a Community Health Nurse (CCHN(C)) is considered an asset.
