Position Overview
The Resource Coordinator for Complex Special Needs plays a pivotal role in ensuring that eligible children receive appropriate services. Reporting to the Supervisor of Complex Special Needs, this position emphasizes a person-centered approach to develop and implement tailored service plans that address individual needs while coordinating necessary services.
Key Responsibilities- Engage with individuals and their families to assess current and future needs, linking them to relevant resources and participating in goal-setting through personalized care plans.
- Develop individualized support plans in collaboration with families and support networks to identify specific requirements.
- Conduct annual reviews of service documentation and ensure compliance with regulatory updates.
- Provide essential resource information to individuals and families as needed.
- Establish and nurture positive relationships with clients and their families.
- Monitor service provision through scheduled and unscheduled visits to clients.
- Address incidents or occurrences related to client care promptly and effectively.
- Participate in clinical consultations and relevant meetings to enhance care provision.
- Manage financial forecasting and contribute to budget planning.
- Report any identified risks related to client care to the Supervisor.
- Collaborate with clients and their representatives to resolve issues as they arise.
- Facilitate discussions regarding financial matters in accordance with departmental policies.
- Ensure the accuracy and integrity of client information in compliance with established standards.
- Attend regular supervision sessions to review case loads and receive guidance.
- Adhere to all organizational policies and procedures.
- Promote awareness of services through participation in presentations to community groups and agencies.
- Perform additional duties as assigned.
- Minimum of three years of experience in the developmental services sector.
- A Bachelor's degree in Social Work or a relevant diploma in Social Work or Support Services is required.
- Strong interpersonal and communication skills.
- Proficient analytical, problem-solving, and critical thinking abilities.
- Familiarity with available services for individuals with developmental needs.
- Experience in case management.
- Knowledge of relevant legislation and regulations governing developmental services.
- Excellent organizational skills and the ability to manage multiple priorities effectively.
- Proficient in Microsoft Office Suite, including Word, Excel, PowerPoint, and Outlook.
- A valid driver's license and access to a vehicle for work-related travel are necessary.
This role involves significant interaction with clients in various settings, including homes and community locations, necessitating local travel. Work schedules and priorities are typically established in advance.
