Vancouver Coastal Health is seeking a dedicated Social Worker to join our Substance Use Intensive Case Management Team in Vancouver, BC.
As a key member of our interdisciplinary mental health team, you will play a vital role in supporting individuals struggling with complex substance use disorders, mental health challenges, and chronic health conditions.
Your Responsibilities:- Conduct comprehensive social and psycho-social assessments to understand the unique needs of each client.
- Develop and implement individualized treatment plans in collaboration with clients, families, and other healthcare professionals.
- Provide direct case management services, including connecting clients with resources, advocating for their needs, and monitoring progress towards goals.
- Collaborate effectively with a multidisciplinary team to ensure comprehensive and coordinated care.
- Contribute to program development and evaluation initiatives to enhance service delivery.
We are seeking a compassionate and experienced Social Worker with a Master of Social Work (MSW) degree from an accredited institution. A Bachelor of Social Work (BSW) with equivalent experience may also be considered.
You possess strong clinical skills, excellent communication and interpersonal abilities, and a commitment to working with individuals facing complex challenges.
