Job Title: Mental Health and Addiction Case Coordinator

Job Summary:

Providence Health Care is seeking a qualified Mental Health and Addiction Case Coordinator to join our team. As a key member of our interdisciplinary team, you will provide flexible and inclusionary treatment and intensive case management services to individuals, primarily youth and young adults, with mental health and addiction issues.

Key Responsibilities:

* Provide outreach services to youth and young adults with chronic psychiatric issues and serious mental illness and/or addiction issues using an intensive case management approach
* Establish, maintain, and enhance therapeutic relationships with clients and their informal support networks
* Provide treatment services, including crisis intervention, family work, education, and support, according to various theoretical models
* Collaborate with interprofessional team members to ensure optimal plan of care is implemented

Requirements:

* Registered Nurse with current practicing registration with the British Columbia College of Nurses and Midwives or a Registered Psychiatric Nurse with current practicing registration with the British Columbia College of Nurses and Midwives
* Two years' recent, related experience working in youth and young adults with mental health and addiction in a community health setting
