We are seeking a highly organized and detail-oriented Patient Care Assistant to join our team at Home and Community Care Support Services. As a key member of our team, you will provide administrative support services to Care Coordinators, process new referrals and orders, and maintain confidential patient records.
Key Responsibilities- Provide administrative support services to Care Coordinators, including processing new referrals and orders
- Enter, update, and maintain patient data in our electronic database
- Answer telephone inquiries from patients, families, and service providers, and refer callers as appropriate
- Provide back-up support to other positions as required
- Grade 12 diploma (minimum)
- 2 or more years' related office experience
- Accurate keyboarding/data-entry skills
- Proficient with database software, MS Word, Excel, and other applications in a Windows environment
- Excellent organizational skills and ability to work independently with minimal supervision
- Advanced multi-tasking skills, with the ability to meet performance and service goals
- Exceptional interpersonal skills and ability to work as part of a team and interact tactfully and sensitively with patients from diverse backgrounds
- Excellent oral and written communication skills, including strong listening skills
- Flexibility to work a schedule that includes days, evenings, and weekends to meet organizational needs
- Ability to provide a criminal background check
