Clinical Nurse

About the Role:
We are seeking a skilled Registered Practical Nurse to join our team in a full-time, temporary 6-month contract position. As a member of our team, you will work with patients of all ages, providing high-quality care and support in a dynamic and supportive environment.
Key Responsibilities:
- Provide ethical and safe nursing care to patients in their homes and communities
- Use assessment tools and techniques to identify individual patient needs and develop care plans
- Collaborate with patients and their families to build rapport and trust, and to engage them in their care
- Maintain accurate and up-to-date patient records
- Participate in scheduled visits and provide care as needed
Requirements:
- Registration with the CNO as a Registered Practical Nurse in good standing
- Valid driver's license and access to a vehicle
- Experience in delivering community care is an asset
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area
