Clinical Nurse Specialist

About the Role:
We are seeking a skilled and compassionate Clinical Nurse Specialist to join our team at ParaMed Home Health Care. As a key member of our nursing team, you will play a vital role in delivering high-quality patient care and making a positive impact in the lives of our clients.
Key Responsibilities:
- Participate in scheduled visits in the community to assess and address patient needs within your scope of practice.
- Provide safe and effective nursing care, utilizing evidence-based practices and protocols.
- Conduct thorough assessments of patients' individual needs, using appropriate tools and techniques.
- Develop and implement personalized care plans, monitoring and adjusting as necessary to ensure optimal patient outcomes.
- Build strong relationships with patients and their families, fostering trust and promoting a collaborative approach to care.
- Maintain accurate and up-to-date patient records, documenting all relevant information and interactions.
Requirements:
- Registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse in good standing.
- Valid driver's license and access to a reliable vehicle.
- Experience in delivering community care is an asset.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Flexibility and reliability, with a willingness to travel to and from patient residences within a designated area.
- Passion for delivering high-quality patient care and making a positive impact in the community.
