Social Worker II

We are seeking a highly skilled and compassionate Social Worker II to join our Primary Care team at Northern Health Authority. As a key member of our interprofessional team, you will provide comprehensive assessment, care planning, and intervention services to individuals and families in our community.
About the Role:- Provide direct psychosocial care, counseling, and group work to individuals and families
- Collaborate with healthcare providers and the interprofessional team to develop and implement care plans
- Facilitate client self-management and provide education on relevant health topics
- Link clients to community resources and specialized services as needed
