We are seeking a highly skilled Social Worker II to join our Primary Care team at Northern Health Authority. As a key member of our interprofessional team, you will provide comprehensive assessment, care planning, and intervention for individuals and families in our community.
Key Responsibilities- Provide direct psychosocial care, counseling, and group work to individuals and families
- Facilitate client self-management and provide education on relevant health topics
- Collaborate with healthcare providers and community resources to ensure seamless care coordination
- Develop and implement care plans that address the unique needs of each individual or family
- Bachelor's degree in Social Work from an accredited university
- Two years of recent experience in healthcare or a related field
- Current registration with the British Columbia College of Social Workers
- Valid BC driver's license
