Job Summary

The Palliative Care Specialist will provide comprehensive care to patients with complex medical needs, utilizing a holistic approach that emphasizes partnership and compassion. This role requires a strong foundation in palliative care, pain and symptom management, and a commitment to delivering exceptional patient outcomes.

Key Responsibilities
  • Conduct thorough health assessments and develop individualized plans of care
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Provide education and support to patients and families on palliative care and end-of-life options
  • Stay up-to-date with best practices in palliative care and pain management
  • Participate in quality improvement initiatives to enhance patient care
Requirements
  • Current registration in the Extended Class (NP-Adult) with the College of Nurses of Ontario
  • Completion of a Master Degree in Nursing
  • Minimum of 3 years nursing experience in palliative care
  • Exceptional knowledge in palliative care and pain & symptom management
  • Formal training and education in Palliative Care with minimum of LEAP Course completion
Preferred Qualifications
  • Eligibility for cross-appointment to an academic institution
  • Membership in specialty related organizations NPAO, RNAO
  • Innovative, creative and professional role model
  • Demonstrated ability to prioritize/organize multiple demands
  • Ability to work autonomously
