Job Summary

We are seeking a highly skilled and compassionate Client Care Coordinator to join our team at Spectrum Health Care. As a key member of our support services team, you will play a vital role in ensuring the highest level of care and service delivery to our clients.

Key Responsibilities
  • Collaboration and Coordination: Work collaboratively with community healthcare professionals, including Personal Support Workers (PSWs) and Nurses, to schedule and coordinate services that meet the unique needs of our clients.
  • Client Liaison: Develop and maintain strong relationships with clients, staff, and community partners to ensure seamless communication and service delivery.
  • Problem-Solving and Support: Act as a problem solver for clients, responding to inquiries and concerns in a timely and professional manner.
  • Reporting and Documentation: Accurately report client and staff concerns to the Home Support Supervisor, and maintain accurate records of client data and service delivery.
  • Administrative Support: Provide administrative support to the personal support worker and Nurse program, including scheduling, data entry, and other tasks as required.
Qualifications
  • Experience: Minimum of 2 years of experience working in an office setting, preferably in a healthcare or social services environment.
  • Availability: Ability to work weekends and flexible hours as required.
  • Communication Skills: Excellent written and verbal communication skills, with the ability to communicate effectively with clients, staff, and community partners.
  • Organizational Skills: Ability to multitask, prioritize tasks, and maintain accurate records.
  • Education: High school diploma or equivalent.
  • Technical Skills: Proficient in Microsoft Office, including Word and Outlook, and able to learn proprietary software as required.
Additional Requirements
  • Medical Terminology: Knowledge of medical terminology is an asset, but not required.
  • Language Skills: Excellent English language skills, with additional language skills considered an asset.
