Care Coordinator

Join our team at Home and Community Care Support Services as a Care Coordinator, where you will play a vital role in providing connected, accessible, patient-centred care to our clients.
About the RoleAs a Care Coordinator, you will be responsible for assessing and determining patient care needs and eligibility, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners.
Key Responsibilities- Collaborate with patients and their families to assess care needs, determine eligibility for services, and develop individual care and service plans
- Link patients with service providers and coordinate care plan delivery
- Establish a helping relationship with patients and their families, balancing their needs and choices with available resources
- Work collaboratively with inter-professional teams, including partners and providers within the circle of care, caregivers, parents, school staff, respite providers, and colleagues
- Membership in good standing with the applicable regulatory body (College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.)
- Three to five years of recent experience in children's health with a focus on children and youth with medical complexity and high risk/high utilization conditions
- Understanding of Ontario's provincial supports and strategies available to children with medical complexity and other conditions
- Experienced and knowledgeable regarding community services and resources for children and youth within Mississauga Halton Geography
- Clinical expertise providing patient and family-centred care, including critical thinking, ethical decision-making, and collaboration with various stakeholders
- Leadership abilities to support inter-professional teams and effective time management, prioritization, and organizational skills
