Job Title: Case Management Worker

We are seeking a skilled and compassionate Case Management Worker to join our team at the Indian Youth Friendship Society. As a Case Management Worker, you will play a vital role in supporting the reintegration of offenders into the community.

Job Summary:

The Case Management Worker will be responsible for assessing the needs of offenders, developing and implementing individualized plans, and providing support and guidance to help them achieve their goals. This is a challenging and rewarding role that requires strong interpersonal skills, a non-judgmental attitude, and a commitment to supporting vulnerable populations.

Key Responsibilities:
  • Conduct assessments to identify the needs and goals of offenders
  • Develop and implement individualized plans to support offenders in achieving their goals
  • Provide support and guidance to offenders, including counseling and advocacy
  • Collaborate with community agencies and services to provide offenders with access to resources and support
  • Monitor and evaluate the progress of offenders, making adjustments to plans as needed
Requirements:
  • Post-secondary education in a related field (e.g. social work, counseling, psychology)
  • Experience working with vulnerable populations, including offenders
  • Strong interpersonal skills and ability to work effectively with diverse groups
  • Non-judgmental attitude and commitment to supporting offenders
  • Ability to work independently and as part of a team
