Position Overview

The Case Coordinator at VON Canada plays a vital role in delivering exceptional client care through thorough assessments and tailored support. This position is integral to the SMILE Program, ensuring clients receive personalized care plans that align with their individual goals.

Employment Details:

- Employment Status: Regular, Full Time (RFT 1.0)
- Number of Hours Bi-Weekly: 75
- Work Schedule: Days
- On-Call: No
- Location: Hybrid remote, with community visits 2-3 times weekly.

Key Responsibilities:

  • Conducts comprehensive assessments utilizing standardized tools, ensuring a high-quality client experience.
  • Develops and implements individualized care plans in collaboration with clients and caregivers.
  • Supports clients in managing their care budgets and accessing necessary resources.
  • Visits clients in their residences to perform case management duties.
  • Identifies and addresses environmental risks impacting client health and safety.
  • Facilitates referrals to community and health services as needed.
  • Monitors client progress and adjusts care plans accordingly.
  • Collects and reports relevant data to management.
  • Upholds privacy policies and standards for community support services.

Required Qualifications:

  • University Degree or College Diploma in nursing or a related field.
  • Minimum of 3 years of experience in home and community care, with a focus on case management.
  • Strong knowledge of community resources and services.
  • Proven ability to build and maintain relationships with external partners.
  • Proficient in using technology for assessments and care planning.

  • Strong communication skills, both verbal and written.
  • Ability to manage multiple tasks and prioritize effectively.
  • Demonstrated leadership and team collaboration skills.
  • Commitment to maintaining confidentiality and professionalism.

  • Availability for weekend assessments.
  • Access to a personal vehicle for business use.
  • Criminal Records Check and Vulnerable Sector Search may be required.
