Key Responsibilities:
  • Conduct comprehensive community mental health assessments and document findings.
  • Assess, screen, and refer individuals to internal CMHA services or community resources.
  • Compile and gather collateral information to support case management.
  • Participate in case disposition meetings to determine outcomes for individuals referred to the agency.
  • Consult with the Mental Health First Response/RAIT Team to prioritize individuals awaiting service.
  • Communicate and coordinate relevant information with agency staff.
  • Attend weekly psychiatry rounds as needed.
  • Provide intervention and brief solution-focused therapy support to individuals experiencing a psychiatric crisis.
  • Respond to urgent situations and link individuals with immediate resources.
Qualifications:

Education: Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W, or a Registered Nurse or Registered Practical Nurse with current registration with the CNO, or a Bachelor's Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W, or a Bachelor's Degree with an applicable college diploma in Addictions and current registration with the C.A.C.C.F.

Experience: Several years of experience working with individuals with serious mental illness.

Requirements: Valid Ontario Driver's Licence and access to a vehicle.

Preferred Qualifications: Proficiency in both official languages.

Working Conditions:

35 hours/week, 8:30-5:00pm. Flexible hours required to meet position requirements.

Salary: Commensurate with experience within the range: $53,927 - $76,754, inclusive of HOOPP.
