Clinical Social Services Specialist

As a vital member of the Critical Care Interprofessional team, the Clinical Social Services Specialist will deliver essential psychosocial support to patients and their families.
Key Responsibilities- Perform thorough social work and psychosocial evaluations on inpatients, utilizing diverse methodologies in alignment with professional standards and clinical protocols.
- Develop, document, and execute comprehensive care plans for hospital discharge, ensuring a safe transition back to community or institutional settings.
- Engage in team meetings, educational interdisciplinary rounds, and training sessions.
- Intervene in high-risk or vulnerable situations to provide necessary support.
- Assist patients and families in adapting to health-related challenges and resolving complex interpersonal issues.
- Educate patients and families about relevant legislation, including the HCCA and Substitute Decisions Act.
- Offer selected therapeutic interventions, supportive counseling, community outreach, and advocacy services.
- Guide patients regarding financial matters, legal issues, workplace injuries, or community referrals.
- Support visitors who require hospitalization.
- Participate in advanced care planning and discussions regarding goals of care through family meetings and facilitated conversations.

- BSW or MSW is required.
- Current registration and membership in good standing with OCSWSSW.
- Experience in providing social work services to patients in an acute care hospital environment.
- Strong clinical competencies in assessment, counseling, and discharge planning/community integration.
- Extensive knowledge of community resources and a proven ability to collaborate effectively with community partners.
- Familiarity with the Mental Health Act, Consent to Treatment Act, Substitute Decisions Act, and Healthcare Consent Act.
- Demonstrated commitment to maintaining an evidence-based practice approach.
- Previous group experience with this population is preferred.
- Commitment to ongoing professional development.
- Ability to document in accordance with college standards.
- Capacity to work independently, think critically, and solve problems effectively.
- Experience in conducting interviews and providing comprehensive psychosocial assessments.
- Strong understanding of the Public Guardian and Trustee Office and Consent and Capacity Board.
- Excellent employment and attendance history.
Work Schedule: Monday to Friday - Day shift and/or evenings (subject to operational requirements).
Compensation: $57.90 per hour.
