About the Role

VHA Home HealthCare is seeking a highly skilled Health Services Scheduling Coordinator to join our team. As a key member of our client care team, you will play a vital role in ensuring our clients receive the highest level of care and support.

Key Responsibilities
  • Coordinate client services, including scheduling, service changes, and updates, using our state-of-the-art scheduling software.
  • Maintain accurate and up-to-date client service plans, ensuring seamless communication with clients, caregivers, and funders.
  • Provide exceptional customer service, responding to client inquiries and resolving issues in a timely and professional manner.
  • Collaborate with internal teams, including nursing and personal support worker coordination desks, to ensure seamless service delivery.
  • Support the development and implementation of client-centered care plans, ensuring the needs of our clients are met.
Requirements
  • Post-secondary school education in a healthcare-related field, preferably with a certificate or diploma in healthcare education.
  • Minimum of three years of related work experience in a healthcare organization or agency, preferably in a community setting or in an administrative role with extensive scheduling experience.
  • Excellent telephone skills, with the ability to interact professionally with clients and stakeholders.
  • Strong organizational skills, with attention to detail and the ability to prioritize tasks and manage multiple responsibilities.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, caregivers, and internal teams.
  • Knowledge of the Greater Toronto Area (GTA) and TTC routes, or relevant contracted areas.
  • Strong computer and data entry skills, with the ability to input data accurately and efficiently.
