Primary Care Physician

Lifemark Health Group is seeking a skilled Primary Care Physician to join our team in Victoria, BC. As a Primary Care Physician, you will play a vital role in providing comprehensive rehabilitation-focused assessment and intervention for Canadian Veterans who are experiencing barriers transitioning to life after service due to service-related injuries or illness.
Key Responsibilities:- Conduct thorough assessments of presenting eligible health problems
- Perform medication reviews and identify drug therapy problems
- Clarify relevant medical issues, including comorbid conditions
- Clarify diagnosis and prognosis related to eligible health problems
- Identify conditions that are not related to eligible health conditions that, if left unaddressed, can hinder progress in rehabilitation
- Identify relevant accommodations or medical restrictions
- Collaborate with Veterans, support persons, family members, and primary healthcare providers to promote understanding of relevant medical conditions and recommendations
- Communicate directly with Veterans' primary healthcare providers to collaborate on rehabilitation care
- Care may be provided in-person or virtually
- Engage in time-bound targeted follow-up care through a consultative model with Veterans receiving rehabilitation treatment within Lifemark Health Group
- Collaborate with Lifemark Health Group Medical Directors and engage in continuous quality improvement, learning, and skills development
- Doctor of Medicine and valid license to practice in the respective field in the province/territory as applicable, and member in good standing with provincial regulatory body
- Experience in the management of complex medical conditions
- Experience with military culture and Canadian Veterans is an asset
- Bilingual (French/English) is an asset
- Experience working with a variety of health professionals and disciplines in an interdisciplinary team environment is an asset
