Job Summary:We are looking for a dedicated and compassionate Primary Care Physician to provide high-quality medical care to Canadian Veterans. The successful candidate will work collaboratively with our interdisciplinary team to assess and manage complex medical conditions, promote rehabilitation, and improve the overall health and well-being of our clients.
Key Responsibilities:- Conduct thorough assessments of presenting eligible health problems
- Perform medication reviews and identify drug therapy problems
- Clarify relevant medical issues, including comorbid conditions
- Identify conditions that may hinder progress in rehabilitation
- Provide recommendations for accommodations or medical restrictions
- Collaborate with clients, support persons, and family members to promote understanding of medical conditions and recommendations
- Communicate with primary healthcare providers to collaborate on rehabilitation care
- Engage in targeted follow-up care through a consultative model
- Participate in continuous quality improvement, learning, and skills development
- Doctor of Medicine and valid license to practice in the province/territory
- Member in good standing with the provincial regulatory body
- Experience in managing complex medical conditions
- Experience working with a variety of health professionals and disciplines in an interdisciplinary team environment
- Experience with military culture and Canadian Veterans
- Bilingual (French/English)
