Job Summary

We are seeking a highly skilled and experienced Discharge Planner to join our team at Halton Healthcare. As a key member of our Social Work, Discharge Planning and Patient Flow team, you will play a critical role in ensuring the smooth transition of patients from our hospital to their community or institutional settings.

Key Responsibilities
  • Develop and implement comprehensive discharge plans that address the unique needs and goals of each patient and their family system.
  • Collaborate with interdisciplinary teams to identify and address high-priority discharge barriers and ensure efficient use of hospital and community resources.
  • Provide education and support to patients and their families to ensure a safe and successful transition to their community or institutional settings.
  • Identify and coordinate referrals to community services and resources to meet the ongoing needs of patients and their families.
  • Advocate for patients and their families to address gaps in services and system-level changes required to meet the changing needs of our patient population.
Requirements
  • Bachelor of Science in Nursing (BScN), Bachelor of Social Work (BSW), or Master of Social Work (MSW) degree required.
  • Current registration and membership in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) or the Ontario Nurses Association (ONA).
  • 2-5 years of clinical experience in an acute care hospital setting, with a minimum of 3 years of experience in discharge planning and patient flow.
  • Current knowledge of community resources and a commitment to proactively seeking out new resources to support patients and their families.
  • Experience with legislation governing public hospitals, including the Public Hospitals Act, Substitute Decisions Act, and Consent to Treatment Act.
  • Expert communication, collaboration, organizational, and problem-solving skills, with the ability to work independently and as part of an interdisciplinary team.
